Thank you for choosing Cascade Health Clinic as your health care provider.
Please fill out the new patient intake questionnaire and email or fax it to our office prior to your first appointment. This helps the doctor get a complete understanding of your unique situation before your visit.
What can you expect from your first visit:
Your first office visit at Cascade Health Clinic is approximately 45 to 60 minutes in length and involves a complete and thorough review of the detailed intake questionnaire, full health history, we go over your health concern, and if necessary, may include a physical exam. In most situations, your provider will craft a thorough treatment plan that may include various treatments such as botanical, homeopathy, and other natural means first. Subsequent visits vary from 15 minutes to 40 minutes in length.
What to bring for your first visit:
- Your completed new patient forms prior to your visit. Be sure to email or fax it over to our office.
- Copies of any blood work or laboratory reports that are recent as of the last six months (or longer if appropriate) and any other pertinent documents, if you have them or we can always request your medical record with a medical release found here.
- If possible, please bring (or a list of) medications or supplements that you are currently taking.
Cascade Health Clinic accepts payment by Debit, Visa, MasterCard, Cash or Personal Check. Most health insurances cover your visits at Cascade Health Clinic but in the event your insurance does not cover your visit, we do have affordable rates. Please check with your individual insurance prior to your visit to determine the amount that is covered under your policy and any co-pays, if any.
