To get started, we ask that you follow these simple steps so that at your appointment we can focus on your medical care and not filling out paperwork and verifying insurance coverage.

Step 1

Patient Intake Form

Please download and fill out the New Patient Intake Form completely. Be sure to sign the first page as well as the Informed Consent page.

Step 2

Insurance Card

We will need a copy of your insurance card both the front and back of the card. If you have a secondary insurance, we will need a copy of the front and back of card as well. We recommend using Adobe Scan – a free app for your phone – that allows you to easily take pictures of your insurance card and turns then into PDF files (we can not accept images).

Adobe Scan Instructions: After you download the app click the image icon and place phone over insurance card. You will not need to press any buttons as the app will take a picture automatically and crop it. Once you have a picture of the front of your insurance, now click the image of your insurance card in the lower right corner. You will then click ‘add page’ which will then allow you to take a picture of the back of your insurance card. You can then rename it (Your Name – Insurance Card) then save it. Lastly, click the ‘share’ button and email to your self. Done!
