Replacement COVID-19 Vaccine Card

We will mail the replacement COVID-19 Vaccine Card usually within 3-7 business days, but could take longer as our vaccine coordinator is extremely busy. Information Not Found in State Immunization Database

If your information is not found in the state immunization record (WA IIS) we can assist you in correcting this. In many instances, you information is there but you may not be searching for it correctly. We usually enter your information from your ID/Driver’s License into the state immunization database. Sometimes people use a different name than their ID/Driver’s License (ie. Robert may go by Bob or Katrine may go by Katie). Please try to search for your record in the state database by using your ID/Driver’s License information first then alternative name spelling that you may use. In most instances, this will resolve your issue. If you are still having issues, you may fill out the Contact Us form and someone will follow-up with you. Any issues related to an app for QR codes or electronic access to your vaccine information should be directed to the app provider or the department of health not us.
