COVID-19 Testing
Yes, we offer COVID-19 testing. If you are experiencing symptoms, please call our office to schedule a visit so that we may prepare to see you. In many cases, we are able to test patients from their vehicle, if requested.
COVID-19 Vaccine
We do not offer the COVID-19 vaccine.
Replace COVID-19 Vaccine Card
If you need a replacement COVID-19 Vaccine Card there is a $20 fee.
Verify WA Vaccine Record
If you need to verify that your COVID-19 vaccination is entered into the Washington Immunization Information System (WA IIS).
